Supporting Falcon Student-Athletes
The Lackawanna College Sports Medicine Department is committed to providing quality healthcare services that support the health, safety, and performance of our student-athletes. Our athletic training staff works closely with team physicians, coaches, and campus partners to deliver comprehensive injury prevention, evaluation, treatment, rehabilitation, and return-to-play services.
Our goal is to help every student-athlete compete safely while promoting long-term health and wellness both on and off the field.
Sports Medicine Services
The Sports Medicine Department provides:
- Injury prevention and risk reduction programs
- Injury and illness evaluation
- Emergency care and injury management
- Rehabilitation and therapeutic exercise
- Return-to-play assessment and clearance coordination
- Coordination of physician referrals and specialty care
- Student-athlete health education
- Pre-participation medical screening and documentation review
Student-athletes are expected to report all athletic-related injuries and illnesses to the Athletic Training Staff as soon as possible to ensure appropriate care and safe participation.
Required Participation Forms
Before participating in practices, workouts, or competition, student-athletes must complete all required Sports Medicine documentation, including:
- Pre-Participation Physical Examination
- Medical History Forms
- Sickle Cell Documentation
- Medical Release Authorization
- Insurance Acknowledgement Form
- Emergency Contact Information
- Standard Operating Procedures Acknowledgement
Required sports medicine documentation is a prerequisite for athletic participation.
Important Note
The Sports Medicine forms and participation packet are distributed by the athletic department and sports medicine staff. If you have not received your required forms, please contact your respective head coach or a member of your coaching staff for assistance. Your coach will provide the necessary documentation and instructions for completion.
Insurance Requirements
All student-athletes must maintain personal health insurance that provides coverage for athletic-related injuries as a condition of participation. Proof of insurance must be submitted before participation in any team activities. Student-athletes are responsible for reporting any changes to their insurance coverage immediately.
Commitment to Student-Athlete Welfare
Participation in collegiate athletics involves inherent risks. The Sports Medicine Department works to minimize those risks through education, prevention strategies, medical monitoring, and individualized care plans. Student-athletes share responsibility for maintaining a safe environment by promptly reporting injuries, illnesses, and medical concerns.
