To begin the eligibility certification process and prepare for participation, please complete the following steps:
Required Eligibility and Enrollment Items
1. Register with the NCAA Eligibility Center
- Create your account and complete the required amateurism and academic information through the NCAA Eligibility Center - https://web3.ncaa.org/ecwr3/.
2. Complete and Submit Required Forms
- NCAA Student-Athlete Statement
- NCAA Drug-Testing Consent Form
- Student-Athlete Information Form
- Waiver of Liability
- Authorization to Release Academic Information
- Photo and Personal Information Release
- Athletics Rules, Regulations, and Code of Conduct
- Sports Medicine Packet (includes physical forms, insurance information, emergency contacts, and medical history)
Note: The forms and requirements listed above are distributed by the athletic department. If you have not received these materials, please contact your respective head coach or a member of the coaching staff for assistance. Your coach will provide all required NCAA eligibility, compliance, and sports medicine documentation and answer any questions regarding the completion process.
